HEALTH FORMS
It is a Deparment of Education requirement that any medication adminstered at school, must first be authorsied in writing by the parent/guardian.
Requests for staff to administer medication must be made at the office. Parents / guardians are asked not to make individual requests to the teacher.
Staff will not administer any medication (short or long term) without written authorisation.
Department of Education policy states:
Medication must be clearly labelled with the child’s name, and provided in the orginal packaging with expiry date from the pharmacy, or the manufacturer.
Documentation must be signed and dated by a parent or carer and provided to the school with the medication.

Student Health Care (Form 1). Parent to complete if students details change and reviewed annually
- Generic Health Care Plan (Form 2) If the child's condition does not match any other plan - e.g. ADHD
- Administration of Medication (Form 3) Short Term - For students who require daily medication while at school e.g. antibiotics, paracetemol
- Anaphylaxis Action Plan - To be completed with your child's photo and returned with (Form 4)
- Severe Allergy/Anaphylaxis - (Form 4) with Anaphylaxis Action Plan this must be signed by local GP.
- Mild and Moderate Allergies Plan (Form 5) For students who have mild allergy to bees, nuts, medication
- Diabetes Action and Management Plan - For further information please see Diabetes best practice guidelines (These plans replace Form 6)
- Seizure Management Plan (Form 7)
- Asthma Management Plan (Form 8) Parents & Doctor to complete for students with Asthma. This must be signed by local GP.
- Activity of Daily Living (Form 9)
- Emergency response plan ESCU.-1 (Form 10)
